If you or anyone in your household enrolled in an AffinityAccess plan through the NY State of Health (NYSOH), this tax season you’ll get a new Form 1095-A — Health Insurance Marketplace Statement from NYSOH . You’ll get this form in the mail and use it to file your 2016 federal income tax return. Keep it with other important tax information, like your W-2 forms and other tax records.
A sample of Form 1095-A is available from the IRS website. When you get Form 1095-A, make sure the information matches your records. If you or anyone in your household enrolled in an AffinityAccess Catastrophic or off-exchange plan, you will receive Form 1095-B in the mail directly from Affinity. Use this form to file your 2016 federal income tax return. Again, when you get your form, check to make sure the information matches your records (sample Form 1095-B).
