    Affinity Health Plan cares about your mental health and well-being. With the goal of improving the physical health of our members with mental illnesses and addictions, Affinity Health Plan is dedicated to:

    • Affinity now covers more comprehensive care for adults 21 years of age and older with serious mental illness who live in any of the 5 boroughs of New York City. This care includes mental health and substance use (alcohol and drugs) treatment and rehabilitation services. Improving access to primary care services,
    • Improving prevention, early identification, and intervention to reduce the incidence of serious physical illnesses, including chronic disease;
    • Increasing availability of integrated, holistic care for physical and behavioral disorders

    Affinity now covers more comprehensive care for adults 21 years of age and older with serious mental illness who live in any of the 5 boroughs of New York City. This care includes mental health and substance use (alcohol and drugs) treatment and rehabilitation services.  Members have access to these services to help with emotional health, alcohol or other substance use issues, including:

    Mental Health Care

    • Intensive psychiatric rehab treatment
    • Day treatment
    • Clinic continuing day treatment
    • Inpatient and outpatient mental health treatment
    • Partial hospital care
    • Rehab services if you are in a community home or in family-based treatment
    • Continuing day treatment
    • Personalized Recovery Oriented Services
    • Assertive Community Treatment Services
    • Individual and group counseling
    • Crisis intervention services

    Substance Use Disorder Services

    • Short-term Inpatient Rehabilitation
    • Inpatient detoxification services
    • Ambulatory detoxification
    • Opioid Treatment Programs including Methadone and Suboxone maintenance
    • Long-term Residential Treatment
    • Outpatient alcohol and drug treatment services Detox services
    • Co-Occurring Disorders (both mental health and addiction services)

    As your health plan, we work with our primary care and mental health providers as they consult with each other and share relevant health information, with your consent.
