  • Follow these seven easy steps to enroll for coverage with an AffinityAccess Plan on the NY State of Health Marketplace.

    1. Create an Account

      This is where you provide your personal information and contact details (email, phone number, home address, and social security number).

    2. Identity Proofing for Account Holder

      ID proofing is used to verify your identity. You’ll need to provide specific information before you can enroll in coverage and get help with costs.

    3. Build Your Household

      Who lives with you in your household? How old are they? What is their relationship to you?

    4. Confirm Your Income

      Based on the information you provided in Step 1, the system may already have your income information. If you think the number is accurate, you can accept it. If not, you can fill in income information based on your pay stubs, statement of wages, a W-2 or your tax returns.

    5. Supply Additional Information

      You will be asked for additional information which will help determine program eligibility. Examples include, but are not limited to, other health care coverage, absent parents for children under the age of 21, enrollment in disability or retirement benefits, and others. If you are eligible for a subsidy, you will need to complete your enrollment on the NY State of Health Marketplace.

    6. Learn Your Eligibility Status

      In this step, you will get an estimate of what your subsidy would be, and find out if you are eligible for premium tax credits to help you buy a Qualified Health Plan (QHP).

    7. Compare and Buy a Health Plan

      The system will display options and premiums based on the income information you provided in Step 3. The plans you see and their prices are based on any tax credits or subsidies you may be eligible for.

  • Here is a list of important documents to have ready when you enroll:

    • A working e-mail address
    • Social security number, proof of citizenship or legal status in the United States
    • Proof of New York residency
    • Proof of date of birth, such as birth certificate or driver's license
    • Income information, such as pay stubs, W-2 forms or wages and tax returns
    • Name of your doctor(s) and medications

    Individuals and households below certain income limits may qualify for premium tax credits to buy any metal level product and for a reduced cost sharing version of the Silver Plan. Government financial assistance is only available when you buy a health plan on the New York State of Health Plan Marketplace.
