  • Member Rewards Program Guidelines

    The Member Rewards Program is available to all Affinity Members whose membership is active at the time services are provided. For a list of medical services, age limits and care that qualify, select a link below:

    Member Rewards Program Tracking Forms
    All information on the Member Rewards Program Tracking Form, including your doctor’s signature, must be completed in order to receive a gift card. A scanned copy of your completed form may be emailed to Affinity’s Quality Management Department at You can also fax your form to 718.794.7821 or mail it to Affinity Health Plan, Attention: QM Department, 1776 Eastchester Road, Bronx, NY 10461.

    Gift Cards
    Gift cards may only be used by Affinity Health Plan members and cannot be converted to cash or used to purchase alcoholic beverages or cigarettes. An annual limit of $125 applies to Essential Plan, Qualified Health Plan, Child Health Plus (CHP), Medicaid and Enriched Health Members. Gift cards will be mailed out within four weeks after Affinity receives your completed Member Rewards Program Tracking Form. If you do not receive your gift card, please email Affinity’s Quality Management Department at or call them at 718.794.7764. Affinity will send a one-time replacement gift card once we have confirmed that the original card was not used.

    Affinity’s Member Rewards Program may be discontinued at any time. If you have any questions, please call Affinity's Quality Management Department at 718.794.7764, Monday through Friday, from 8:30 AM to 5:00 PM.

    Medicare Members
    According to the Centers for Medicare & Medicaid Services (CMS), Affinity must send you your gift card no later than 12/31/17. Please send in your completed Rewards Tracking Form by 11/30/17 so that we can process your reward request.
