  • Welcome New Members!

    Now that you’ve enrolled in an AffinityAccess, AffinityAccess Direct, or AffinityAccess 2.0 plan, here are some next steps you will need to take during the first few weeks of your new membership:


    1. Pay Your Premium

      When you first enroll in your plan, there is a 10-day grace period before your first payment is due. This means that you will have until the 10th day of the month your coverage begins to make your first payment. If your first payment is not received within 10 days, your plan may be cancelled. For all other months your payment is due by the 1st of every month. Payments can be submitted online via the Member Portal. View more payment options here. If you fail to make your monthly premium payments during the plan year, your coverage may be terminated retroactively to the last date to which a payment applies.

    2. Choose a PCP

      If you don’t choose a PCP for yourself and your family within 30 days of enrollment, one will automatically be chosen for you. You may choose any PCP found in the AffinityAccess Network. Each covered family Member can have a different PCP, or you can choose one PCP to take care of the whole family, like a family practice doctor who can treat everyone or a pediatrician just for the kids. The choice is always yours.

    3. Coverage Effective Date

      Your enrollment is not effective until Affinity receives your first premium payment and sends a confirmation to New York State. The date when both actions are complete will be considered your “Coverage Effective Date.”

    4. Welcome Package Delivered by Mail

      Once payment has been received, Affinity will mail you a welcome letter along with your Member ID Card and Member Handbook.

    5. ID Cards

      Your ID Card should be regarded as temporary until you’ve selected a PCP. Once received, detach it, carry it in your wallet or purse and be sure to present it when visiting a health care facility or your doctor.
