Mammography Incentive Program Affinity Health Plan's Mammography Incentive Program reaches out to our female Members who are a) over 50 years old, b) have been enrolled in Affinity Health Plan for a year, but, c) according to our records, have not had a mammogram during the past two years. When we identify a Member who needs a mammogram, one of our Clinical Outreach Specialists contacts her by phone or mail. The Outreach Specialist verifies whether or not the Member has had a mammogram. If she has not had one, the Outreach Specialist then helps the Member schedule the mammogram at one of our participating radiology facilities. After the Member has her mammogram, Affinity sends her a $25 "Supercertificate" issued by GIFTCERTIFICATES.COM as a reward for making the effort to have this critical screening exam. The Member may redeem the certificate at any of 500 participating stores and merchants. Postpartum Visit Program The goal of Affinity Health Plan's Postpartum Visit Incentive Program is to encourage Members to have a postpartum examination between three to eight weeks after delivery. This program is part of our Healthy Beginnings program, which provides care management services for pregnant Members. One of our Clinical Outreach Specialists contacts Members after they give birth. The Outreach Specialist encourages the Member to schedule her postpartum visit. The Outreach Specialist may arrange for transportation to the doctor's office, if necessary, and may make a reminder call to the Member before the scheduled postpartum visit. Members who keep their postpartum visit appointment receive a $25 "Supercertificate" from GIFTCERTIFICATES.COM as a reward for attending to her postpartum care.
