Dental Services

Affinity Health Plan provides dental services through the HealthPlex Dental Network, as follows.

• Medicaid, Child Health Plus and Family Health Plus Members receive a dental card that lists their primary dentist.
  
  
• The Member may call the HealthPlex Help line at 1-800-468-9868 to choose or to change their primary dentist. They can also call for help making an appointment.
  
  
• Members do not need a referral or authorization from their Primary Care Provider for dental services.
