Who can join our Family Health Plus program? People who...

• are over 19 and under 65 years old;
• are permanent residents of New York State who live in New York City, or in Nassau, Orange, Rockland, Suffolk or Westchester county;
• meet the program's income eligibility requirements (see below);
• are U.S. Citizens or qualified aliens;
• are not eligible for Medicaid or Medicare; and
• do not have health insurance.

What are the income eligibility requirements?

Income eligibility varies depending on family size and whether or not the adult applicant has children.

How much does Family Health Plus Membership cost?

This program is free...the State...not the Member...pays Affinity Health Plan. Members do not have to pay a monthly premium. Members are required to make co-payments when receiving certain medical care and services.
