Affinity Health Plan wants to help you have a longer and healthier life! But, we need your help.

Take a few minutes to read about some of the special programs that Affinity has created to help you and your family get the healthcare that you need.

Then, use the information in this Health Pack and your Member Handbook to:

  1. Fill out and mail in the free Affinity Health Assessment Form below.
  2. Make an appointment with your Primary Care Provider right away for your yearly check up.
  3. Make an appointment for your yearly dental check up right away.
  4. Call Affinity if you or any member of your family is pregnant, or has an illness like asthma, diabetes, HIV/AIDS or any other special needs.
  5. Get the mental health care that you need.
