Coverage Programs

Affinity Health Plan offers several health care coverage programs.

You can qualify for one of our Medicare Advantage programs if you live in one of the five boroughs of New York City and are eligible for Medicare and for government payment of some of your Medicare cost-sharing.
Our Medicaid managed care program is open to Medicaid beneficiaries who are eligible through the TANF (Temporary Assistance to Needy Families), Transitional Medical Assistance (formerly Home Relief), or SSI (Supplemental Security Income) programs.
Our Child Health Plus program is open to uninsured children under the age of 19 who are not eligible for Medicaid or Medicare.
Our Family Health Plus program is open to low-income uninsured adults who are not eligible for Medicaid or Medicare.
We offer a Family Planning Benefit program in some parts of our service area to Child Health Plus and Family Health Plus Members enrolled in other health plans that do not provide family planning benefits.
We also operate our UniCare Program for uninsured individuals and families. This program, however, is closed to new enrollment.
