  • How do I renew my coverage?

    Affinity wants to make sure your coverage continues without interruption. Call us toll free at 866.243.3174 or visit any of our Community Service Centers (CSC) near you to find out how and when to renew your coverage. We’ve also provided the names and phone numbers of staff members who can help you recertify at many of our Community Service Centers (CSC).

    • Depending on how you enrolled for health coverage last year, you will receive a letter reminding you to renew your coverage from either your local Medicaid office, from Affinity, or from the New York State of Health Marketplace.
    • If you enrolled in Medicaid, Child Health Plus (CHP) or a Qualified Health Plan (QHP) through the NY State of Health Marketplace (NYSOH) you will need to go back to the NYSOH Marketplace to renew. You can do this by contacting an Affinity representative in your area, logging into your Marketplace account, or calling the NYSOH Marketplace at 855.355.5777.
    • If you qualify to enroll in the new Essential Plan (EP) or were automatically enrolled into the Essential Plan (EP), you will still need to select a health plan. Give us a call or visit one of our Community Service Centers (CSC) so that we can assist you with the transition.
    • Be sure to bring all required renewal forms and other documents to any of our Community Service Center. You may also call Affinity and ask to meet with your Renewal Representative. Our staff can come to your home or meet you somewhere convenient such as a nearby hospital or doctor’s office.

    Medicaid Members

    • If you experienced a life-changing event, like a marriage, birth of a child, or loss of employment, you must notify the state.
    • Contact Affinity as soon as you get your renewal letter and we will help you fill out your Medicaid renewal forms. Can’t find your Medicaid renewal package? Don’t worry! We can help you print your renewal documents via Access NYC.
    • Determine if you can keep your current Medicaid health insurance coverage or otherwise evaluate other low-cost options such as an Essential Plan (EP) or a Qualified Health Plan (QHP).

    Child Health Plus Members

    Essential Plan Members

    • New Yorkers with a low or moderate income have a more affordable healthcare option available to them. The Essential Plan, provides the same essential health benefits as Medicaid or any Qualified Health Plan. And starting in 2017, Affinity will offer Essential Plans with dental and vision coverage for those who qualify.
    • If you have received a renewal letter from the New York State of Health, make sure to contact an Affinity customer service representative at 866.243.3174 for assistance renewing your coverage.
    • In order for your selection to be effective on January 1, 2017, you must enroll between November 16, 2016 and December 15, 2016.
    • Transitions from other programs into the Essential Plan are allowed all year-round. In order for your selection to be effective on January 1, 2017, you must enroll between November 16, 2016 and December 15, 2016.

    Qualified Health Plan Members

    • The renewal period for Qualified Health Plans runs from November 1, 2016 to January 31, 2017. You must renew by December 17, 2016 in order to avoid a lapse in coverage in January 2017. If your coverage is renewed before December 17, 2016 your new coverage will start January 1, 2017.
    • Call us toll free at 866.243.3174 or visit any of our Community Service Centers (CSC) near you to find out how to renew your coverage. We’ve also provided the names and phone numbers of staff members who can help you recertify at many of our Community Service Centers (CSC).
    • If you signed up for an AffinityAccess Qualified Health Plan last year, your current coverage may be automatically renewed, as long as you continue to be eligible for coverage without any changes. Please call us as soon as you receive your renewal letter to verify and avoid a lapse in coverage.
    • You have the option to choose another AffinityAccess Qualified Health Plan policy. Review your benefits coverage to make sure your plan still fits your needs.
