  • Affinity Health Plan now offers cost-effective alternatives to current approved Medicaid services.

    Affinity Health Plan now provides our Medicaid and Enriched Health members with access to alternative services that are more cost-effective and medically appropriate than those regularly offered by your Medicaid plan. Called "In lieu of services" (ILS), this new benefit provides services in settings that are not included or otherwise covered by your plan. Click here to learn more about ILS.

  • Children’s Medicaid Home and Community-Based Services

    Affinity's Medicaid Offers Home and Community-Based Services (HCBS) for Children.

    Medicaid provides HCBS services for children who 1) need extra care at home and/or in the community, and 2) want to avoid going to the hospital or a long-term care facility.

    HCBS offer personal, flexible services to meet the health, mental health, substance use treatment and developmental needs of your children, supporting them as they work toward goals and achievements.

    Chiropractic services are not covered by Medicaid.

    Transportation Services

    Non-emergency transportation services which includes personal vehicle, bus, taxi, ambulete, and public transportation to medical appointments are now administered through New York State. You or your provider must call the vendor listed below to arrange transportation:
