One of our newest tools is Affinity Health On Demand!, an online video education center with a collection of short informational videos on your plan benefits, how to access services on your own (e.g., printing your temporary ID, paying a bill, etc.) and important health topics. Create an account and receive health and member services related videos based on your personal profile.

      Member Rewards Program
      Affinity wants to encourage the overall health of our members and increase the quality of care in order to improve health outcomes. To this end, Affinity Health Plan has launched a new Member Rewards Program that offers two different incentives:
      HRA: Members will receive a $25 gift card after completing a Health Risk Assessment (HRA) Survey within 60 days of becoming a member. The survey results lets Affinity assess members’ specialty care needs.
      PCP Visit & Survey: Because providers play a pivotal role in managing the health of our members, Affinity offers an additional $50 gift card after members complete an annual PCP visit and submit a completed survey within 30 days of the visit.
