      Welcome to your member portal. Make yourself at home while you explore all of the options available to you. One of our newest tools is Affinity Health On Demand!, an online video education center with a collection of short informational videos on your plan benefits, how to access services on your own (e.g., printing your temporary ID, paying a bill, etc.) and important health topics. Create an account and receive health and member services related videos based on your personal profile.

      Virtual office hours are Monday to Friday, from 9:00 a.m. to 5:30 p.m. Once connected, you may experience a short wait depending on the number of people ahead of you.

      If you don’t have the Zoom app, visit Zoom.us to download it now. Or you can join without downloading the app by clicking on “Join from your browser” on the webpage that pops up when you open the link we send you. You will then be in the virtual "waiting room." 

      To apply for coverage over the phone, please call 866.694.9815. For renewal assistance, please 866-243-3173. You may also call the NY State of Health at 855.355.5777 or visit www.nystateofhealth.ny.gov.

      For any other questions, call our Customer Service line at 866.247.5678 (TTY 711) Monday to Friday, from 8:30 a.m. to 6:00 p.m. Calls may be monitored for quality assurance.

      The Member Advisory Council (MAC) and the Youth Advisory Council (YAC) offer  members the opportunity to voice their opinions and concerns about their Affinity coverage, benefits and services, helping  Affinity better understand members’ needs. Council meetings are currently held via Zoom.
